All workplace injuries must be reported immediately to your supervisor and the Report of Injury Packet must be completed and submitted within 24 hrs to the business office. As a reminder, injured employees can seek medical treatment at ANY emergency department they wish. This does not apply to Urgent Care facilities. Currently, the only approved urgent care provider is Lake Region Urgent Care in Honesdale, PA. Please review the provider list for all approved facilities listed in the Report of Injury Packet.
