Family Medical Leave (FMLA)
If you are in need of an unpaid medical leave and meet the requirements under the Federal Family and Medical Leave Act of 1993 please complete the attached forms and return them to the attention of Carol Skrypek at 570-296-1811 or email@example.com.
Once your paperwork is received it will be reviewed by administration for approval.
Things to consider prior to an unpaid leave request include:
> sick, personal, and vacation days available to you
> application to your union sick bank (if applicable)
