If you need of unpaid medical leave and meet the requirements under the Federal Family and Medical Leave Act of 1993, please complete the attached forms and return them to the District Office att: Tammy Phipps. For more information, please email tphipps@dvsd.org or call 570-409-2014. 

Once your paperwork is received, it will be reviewed by the administration for approval.

Things to consider prior to an unpaid leave request include:

• sick, personal, and vacation days available to you
• application to your union sick bank (if applicable)
