Volunteer Personal Data Sheet Please review School Board Policy #916 before applying to become a school volunteer. After reviewing the policy, please follow the links below to acquire your clearances. All clearances must be less than a year old. If you will be volunteering 10 hours or more weekly, a TB test is necessary and results must be less than a year old. Complete the Volunteer Personal Data Sheet and bring all forms to the building administrator in the building you intend to volunteer. Only copies of clearances are to be submitted.
