APPLICATION FORM
To support and speak on
behalf of children and youth in the schools, in the community, and before
governmental bodies and other organizations that make decisions affecting
children.
To assist parents in developing the skills they need to raise and protect their
children.
To encourage parent and public involvement in public schools of this nation.
The DVES PTA would like to extend an
invitation to you to join us. The
PTA sponsors many activities to all students during the school year.
By joining the PTA, you are giving us your support for these many events.
The cost is $6.00 per person for the 2008-2009 school year. Simply fill out this application and return it to school
along with a check made payable to DVES PTA, INC.
If you are interested in helping out with any of our scheduled events,
you may indicate so on the back of this form.
Please note that you need not be a member to help in any way with PTA,
but we do encourage you to join. Thank

indicate which ones you feel you may be able to help with. We will compile a
list and may call upon you to help with that event. We do need as many people as
possible to help make these activities happen. Any
