PLEASE SHARE THIS INFORMATION WITH OTHER FAMILIES WHO HAVE CHILDREN AGE APPROPRIATE FOR NEXT YEAR’S KINDERGARTEN CLASS. DVES KINDERGARTEN REGISTRATION STUDENTS MUST BE FIVE (5) YEARS OF AGE ON OR BEFORE OCTOBER 1, AND RESIDE IN THE DELAWARE VALLEY ELEMENTARY SCHOOL AREA IN ORDER TO REGISTER. Parents who wish to have their child attend next year’s kindergarten and have NOT received any previous correspondence from us regarding Library Hour must call Robin Adelfio, Delaware Valley Elementary School Secretary, at (570) 296-1823 between the hours of 9:00-12:00 and 1:00-3:00. If you wish to have your child complete the registration process as scheduled, it is important that you contact Robin Adelfio as soon as possible. Robin Adelfio will need the following information from you when you call: Child’s legal name (last, first, middle), social security number, sex, date of birth, city/state of birth, natural/adoptive father’s first and last name, natural/adoptive mother’s first and last name, mailing address and telephone number. Kindergarten packets will be mailed to parents during the last week of January, . Please complete the forms enclosed in this packet and return them to the Delaware Valley Elementary School Office along with your child’s original birth certificate, immunization records, social security card, and two proofs of residency including one photo ID (eg. driver’s license, landlord affidavit, deed, mortgage statement, tax bill) during the two weeks prior to your child’s kindergarten registration date. IF THE PROPER DOCUMENTATION HAS NOT ALREADY BEEN COPIED OR IS NOT IN HAND AT THE TIME OF REGISTRATION, YOUR CHILD WILL NOT BE REGISTERED. Children entering kindergarten must have the following:
